Upon registration, you will receive an invoice by email with options and instructions for payment. We accept these payment forms: government
purchase orders, government training requests, credit cards, and checks.
Confirmations and directions will be mailed, faxed, or emailed to each participant prior to the training date.
Registered participants must cancel in writing at least 48 hours prior to the training date to avoid a no show fee equal to the total
tuition. A substitute may be made at any time, but the new participant is required to submit this registration form.
